Why Indians use lower BMI thresholds

South Asians tend to carry a higher proportion of body fat — and more of it as visceral fat around the organs — at any given BMI than European-ancestry populations. The consequence is that insulin resistance, type 2 diabetes, fatty liver and cardiovascular disease appear at a lower body weight.

WHO's Asia-Pacific guidance and Indian consensus statements therefore lowered the action points to 23 for overweight and 25 for obesity. Using a Western chart in India systematically reassures people who are already carrying metabolic risk.

What BMI does not tell you

BMI is a screening tool, not a diagnosis. It cannot distinguish muscle from fat, so it misclassifies muscular people as overweight. It says nothing about where fat sits, which for metabolic risk matters more than how much there is — waist circumference is often the better number, and for Indians the usual action points are about 90 cm for men and 80 cm for women.

A BMI in the “normal” band with a large waist and poor blood markers is a real and common pattern in India. If that describes you, the BMI number is the least interesting thing on your report.

What BMI has to do with weight-loss medication

BMI is one of the criteria used to decide whether GLP-1 therapy is appropriate — typically from 27 with a weight-related condition, or 30 without one, with Indian practice often applying lower thresholds for the reasons above. It is a starting filter, not the decision: blood markers, history and contraindications matter more.
