A clear 2026 guide to Wegovy (semaglutide) side effects in India — the common GI effects, why slow dose titration matters, the boxed warning, and when to call a doctor.

Quick answer: The most common Wegovy (semaglutide) side effects are gastrointestinal — nausea, vomiting, diarrhoea and constipation. They're usually mildest when the dose is increased slowly, are strongest in the first few weeks, and ease for most people within a month or two. Serious effects are rare but real, and semaglutide carries a boxed warning for thyroid C-cell tumours — which is why it's a prescription medicine that a doctor must screen you for first.

What are the most common Wegovy side effects?

Because Wegovy is a GLP-1 medicine that slows stomach emptying and reduces appetite, most side effects are digestive. The frequently reported ones include:

  • Nausea — the single most common, especially after a dose increase
  • Diarrhoea or constipation
  • Vomiting
  • Indigestion, bloating, burping
  • Fatigue and headache in the early weeks
  • Injection-site reactions — mild redness or itching

Most are dose-dependent and temporary. They tend to peak in the days after stepping up to a higher dose and settle as your body adjusts.

Why does slow dose titration matter so much?

Wegovy is started low and increased gradually over several weeks for a reason: a slow titration is the most effective way to keep nausea and other GI effects manageable. Rushing the dose is the most common reason people feel unwell. This is exactly why self-dosing without medical supervision is risky — a clinician adjusts your schedule to your tolerance. If you're comparing molecules, see our Mounjaro vs Wegovy guide and the semaglutide in India guide.

How do you reduce Wegovy side effects?

Practical ways to ease nausea and GI effects

  • Eat smaller portions and stop when you feel full
  • Favour bland, lower-fat foods when nausea is strong
  • Stay hydrated — important if you have diarrhoea or vomiting
  • Don't skip your protein; it protects muscle as you lose weight
  • Tell your doctor before increasing the dose if you're still struggling

What are the serious risks to know about?

Serious side effects are uncommon but you should be aware of them. Semaglutide carries a boxed warning for thyroid C-cell tumours (seen in rodent studies), and rare risks include pancreatitis, gallbladder problems, kidney issues from dehydration, and low blood sugar (especially if combined with other diabetes medicines). Anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 should not take it. A doctor screens for all of this before prescribing.

Seek medical help for severe or persistent abdominal pain (possible pancreatitis), signs of an allergic reaction, or symptoms of dehydration. Wegovy is a Schedule H prescription medicine — never source it without a valid prescription.

Are the side effects worth it?

For many people, yes — the GI effects are temporary while the weight-loss benefit (around 15% of body weight in trials) is substantial. But "worth it" is a clinical decision based on your BMI, health conditions and tolerance. The safest first step is a doctor-led assessment. "Side effects are mostly a titration problem, not a dealbreaker. Go slow, eat smaller, stay hydrated, and most people sail through the first month." — MetaFit Medical Team
