Quick answer: Wegovy and Ozempic are both semaglutide, made by the same manufacturer. Ozempic is the type 2 diabetes formulation, dosed up to 2.0 mg weekly. Wegovy is the weight-management formulation, dosed up to 2.4 mg weekly. Same molecule, different approved use and maximum dose.
Same drug, two products
This confuses almost everyone, and the confusion is understandable — the active ingredient really is identical. What differs is the licensed indication and the top dose. Ozempic was developed and approved to treat type 2 diabetes, with weight loss as a well-documented additional effect. Wegovy was developed and approved specifically for weight management in people with obesity, or with overweight plus a weight-related health condition.
What the evidence says for each purpose
For weight management, the key trial is STEP 1, in which semaglutide 2.4 mg produced a mean 14.9% body-weight reduction over 68 weeks in adults without diabetes. For people who have type 2 diabetes, STEP 2 studied the same 2.4 mg dose and found smaller weight reductions than in people without diabetes — an important expectation to set if you are diabetic.
The cardiovascular evidence sits with semaglutide as a molecule: in SELECT, it reduced major adverse cardiovascular events by 20% in people with overweight or obesity and established cardiovascular disease without diabetes.
Why "just use Ozempic for weight loss" is complicated
It happens, and in some markets it happened at scale — which contributed to shortages that left people with diabetes unable to fill prescriptions they depended on. Beyond the supply-ethics problem, the practical issue is that the maximum diabetes dose is lower than the weight-management dose, so you may be treated at a dose below what the weight-loss evidence is built on.
Which product you are prescribed should be a clinical decision based on your diagnosis, not on what a pharmacy has in stock or what a social media post recommended.
Generic semaglutide changes the picture
Semaglutide's patent expiry in 2026 brought a large number of generics to the Indian market at substantially lower prices. That has made access far easier, but it puts more weight on getting the prescription and the supervision right — the molecule being cheaper does not make it a consumer product.
What actually matters more than the brand
Whichever product you are on: dose escalation should be gradual to limit nausea, protein intake and resistance training should protect your muscle while you lose fat, and you should have a maintenance plan before you start, because stopping tends to reverse the result. In the STEP 1 extension, participants regained about two-thirds of their lost weight within a year of withdrawal.
